Where is the answering form?

In the Instructions for Solving section "How to submit answers?", it is written :

For sending your answers, please use the prepared answering form.

This will be the first time that I participate to the contest, and I don't know where to find the answering form on the site.

So I kindly ask some experienced participant to tell me where I can find the answering form.
